Family Support
Ability Health identifies that family support is a key factor in resident rehabilitation. We ensure that the immediate family is involved in every step of the rehab process.

Initial assessment of the resident usually commence with 24 hours of the referral. It is not our practice to inform the family members about the initial assessment but any family member requested to be present for the initial assessment then we would schedule the assessment to their convenience. Physiotherapist provides a physiotherapy information leaflet, business card with all the contact information and a resident satisfaction survey form with the resident. Following the initial assessment with the permission of the resident, physiotherapist contacts the responsible family member and discusses the Physiotherapy plan and intervention.

If for any reason, a family member, next of kin or a Power of Attorney requires particular information, the physiotherapist or the assistant is available by telephone, e-mails, walk-ins, fax, notations in the communication books, written messages etc. The queries will be attended to promptly both within and after normal business hours.

Family members are kept aware of the physiotherapy activities through the following venues:
  1. Email or text messages.
  2. Telephone calls
  3. Care Conferences which are attended by all team members
  4. Communication book - family concerns can be written down and they will be addressed promptly.
Physiotherapists and PTAs are available during the working hours for family meeting with an appointment. These meetings are scheduled with priority to the convenience of the family member. Physiotherapists are available on telephone 7 days/week.
