Resident Care
MOHLTC identifies that only less than 70% of the resident population in a Long Term Care Home required Physiotherapy care, whereas Ability Health is committed to provide all the residents that require Physiotherapy service. We can guarantee a minimum of 3 visits per week for all the Long Term Care residents that require the rehabilitation. And episode of care up to 5 visits/week until the acute episode resolves. All the CCP residents will get 5 visits/week.

Physiotherapists and Physiotherapy Assistants will be assigned according to the number of beds in the home. Ability Health guarantees that apart from the assigned hours in the home, Physiotherapist will be available on the phone for 7 days/week. All the new assessments in the LTC will be attended within 72 hours of referral and within 24 hours for the CCP resident referral. Email and fax referrals are received by Physiotherapists 24 hours/day, 7 days a week.

All the staffs are covered by professional liability insurance, WSIB and short & long term disability Insurance. Physiotherapy staffs are avail to attend all the required facility meeting, such as fall prevention team and less restrain team. They are responsible to complete all the requirements of RAI MDS including care plan.

Assessment, Management & Discharge
During the initial assessment, Physiotherapist determines the short and long term goal of the treatment and the duration to meet that goal. The duration of short and long term goal is determined by the chronicity of the condition. At the end of the set duration to meet the goal, Physiotherapist re-evaluates the resident and either discharge or changes the treatment protocol and set a new goal. Discharge process has three stages:
  1. Communicate the discharge plan with Resident, POA, Physician, registered Nurse and other health care providers those who directly involved in the care.
  2. Teach, illustrate and provide written instructions of the home program.
  3. Follow up assessment in person if the residents continue to reside in the Home, if not by phone.
Ability Health has developed a client survey form to evaluate the residents’ and their POAs’ level of satisfaction with our service. This form is given at the time of the initial assessment to all the residents and POA. The completed form can be mailed or faxed to Ability Health. Client satisfaction survey also can be emailed to our email address.. It is the mandate of Ability Health to ensure that all the residents have received the best care and they are satisfied with that.

Other free of charge Services to all resident


  1. Assistive Device Assessments.
  2. Wound care using pulsed UV lamp.
  3. Acute episode of pain management using Acupuncture up to 3 visits.
  4. Simple Orthotics and splints (OTC)
  5. Exercises equipment & its maintenance.

Reporting


Steel City Physiotherapy provides the following reports to the facility. We are glad to entertain any additional requirements in reporting by Shalom Village.

Invoicing and Payment


Detailed monthly invoice will be provided to the Facility Management. Terms of Payment and its logistics can be finalised on a mutually agreed contract. Fee for Service to residents will be invoiced to the residents directly or as per the standing instructions by the power of attorney of the resident.
